Great Guana Cay is a barrier island located in the Abaco Islands, Bahamas. It is situated approximately 15 miles northeast of Marsh Harbour.

Access to Great Guana Cay is primarily via ferry or private boat. Ferries depart from Marsh Harbour, Abaco, with a journey time of approximately 30 minutes. There is a public dock on the western side of the island. Limited vehicle rental is available on the island, with golf carts being the primary mode of transport. Parking is available at the ferry dock and designated areas near establishments.

Great Guana Cay's geological origin is a combination of coral reef development and sand accumulation over millennia. Historically, it served as a settlement for early Bahamian inhabitants and later became a popular destination for fishing and recreation.

A notable feature is the "Nipper's Beach Bar," a well-known establishment that hosts regular events. The island's name is believed to derive from the sound of the wind whistling through the casuarina trees, resembling a "guana" sound.
