The fort is constructed primarily from granite blocks. Its walls are approximately 10 meters high and feature crenellations. The main structure is rectangular with bastions at each corner. The exterior is weathered stone, exhibiting shades of grey and brown. A central courtyard contains a two-story administrative building and a chapel.

Fort São Sebastião is situated 2 kilometers west of Nampula's city center. Access is via Avenida Eduardo Mondlane, which transitions into Estrada Nacional 103. Public transport options include local minibuses (known as "machimbombos") that run along Avenida Eduardo Mondlane; disembark at the intersection with Rua da Fortaleza.

A local legend suggests that during periods of siege, a secret tunnel connected the fort to a nearby river, allowing for clandestine resupply missions. No archaeological evidence has confirmed this tunnel's existence.
