The gate is constructed from large, roughly hewn stone blocks, primarily granite and limestone. It features a single arched passageway, approximately 3 meters wide and 4 meters high. The stonework shows signs of age and weathering, with moss and lichen present on many surfaces. The top of the gate structure is largely absent, with remnants of defensive walls extending outwards.

The Gate of the Old Town is situated at the eastern end of the causeway connecting the Nessebar peninsula to the mainland. It is approximately 30 kilometers southeast of Burgas city center. Vehicle access to the Old Town is restricted; parking is available in designated lots on the mainland before crossing the causeway. Public transport options include bus lines from Burgas to Nessebar, with a stop near the causeway entrance.

The current visible structure of the gate dates primarily from the medieval period, likely the 13th or 14th century, though its origins are believed to be much older, possibly dating back to Thracian fortifications.
