The basilica is constructed primarily of limestone and granite. Its exterior features a large central dome, inspired by St. Peter's Basilica in Rome, and two smaller flanking bell towers. The facade is adorned with numerous statues and mosaics. The interior is characterized by extensive use of marble, mosaic artwork covering over 100,000 square feet, and 80 chapels and oratories dedicated to various saints and devotions. The main dome reaches a height of 237 feet.

Construction of the basilica began in 1920 and was completed in 1959, with final interior decorations added later. The design was overseen by architects John Russell Pope and later by his associates, Maginnis & Walsh. The original purpose was to serve as a national sanctuary dedicated to the Blessed Virgin Mary under the title of the Immaculate Conception.
